Nearly all British doctors have given placebos

LONDON, March 22 — Some 97 per cent of British family doctors admit giving patients placebos, and more than three-quarters say they prescribe them at least once a week, a survey released yesterday found.

Researchers at the English universities of Oxford and Southampton found that the vast majority of general practitioners (GPs) had used "impure" placebos, or unproven treatments.

These include low doses of effective medicines, antibiotics for suspected viral infections, or nutritional supplements not shown to help a particular condition.

Some 12 per cent of doctors had used "pure" placebos, such as sugar pills or saline solutions, which contain no active ingredients at all, according to responses from a representative sample of 783 doctors surveyed online.

Administering placebos in general practice contradicts ethical guidelines set out by the General Medical Council, Britain's regulator for doctors.

Placebos were given to "induce psychological treatment effects" because patients pushed for treatment or to reassure them, the doctors told researchers, while the study added that levels were similar to that found in other countries.

But more than 90 per cent of doctors objected to placebo use where it could endanger trust between patient and doctor, while more than 80 per cent said they opposed it where deception was involved.

"This is not about doctors deceiving patients," said Jeremy Howick, co-lead author of the study.

"The study shows that placebo use is widespread in the UK, and doctors clearly believe that placebos can help patients."

Placebo use outside clinical trials—where they are used for comparison purposes to test the effectiveness of a treatment, and where patients give informed consent—is generally considered unethical.

A 2008 US survey focusing on rheumatologists and internal medicine practitioners found that about half used placebos, in contravention of guidelines.

The British Medical Association, the professional body for doctors, said it opposed such use.

"The BMA believes there are fundamental problems associated with doctors prescribing placebos. And in our view the unacknowledged use of placebos for patients with capacity is unethical," a spokesman said.

"Prescribing and administering a placebo must entail some degree of patient deception because to maximise the placebo effect, a patient needs to believe that the 'dummy' treatment administered is real.

"Deceiving a patient, even where the doctor is acting in his or her best interests, obviously undermines trust and risks damaging the doctor-patient relationship."

But Howick, of Oxford University, said ethical guidelines should be revisted in light of "strong evidence suggesting that doctors broadly support their use".

Some 66 per cent of respondents to the survey said that pure placebos were ethically acceptable under some circumstances, but 33 per cent said they were never acceptable.

Impure placebos, which can also include unnecessary physical examinations or blood tests, were deemed acceptable by 84 per cent of doctors, according to the research published in open access journal PLOS One. – AFP-Relaxnews

Mulberry warning fuels fears of drop in tourist spending

Other luxury brands have also seen a slowdown in London trade as a result of fewer tourists. - Reuters picLONDON, March 22 — British handbag maker Mulberry issued a profit warning today, partly blaming a drop in spending by tourists who have been crucial to the luxury industry's ability to cope in a flagging European economy.

Danish luxury stereo and television maker Bang & Olufsen also cut its revenue outlook on Friday, adding to jitters in the sector.

Shares in Britain's largest luxury brand Burberry, which like Mulberry relies heavily on tourists in its London stores, dropped as much as 5 per cent, among the biggest declines by a European blue-chip company.

At 10.30am, Mulberry shares were down 17 per cent and Bang & Olufsen's off 12 per cent.

Luxury brands have largely coped with an ailing European economy thanks to strong demand from Chinese buyers, both in their home market and when travelling abroad, although there have been periodic signs of weakness in recent months.

Mulberry, which sells Bayswater handbags for around £1,400 (RM6,500) and mint green cotton tweed coats for 1,750 pounds, also issued a profit warning in October, blaming a slowdown in Asian demand.

The group said on Friday pretax profit for the year ending March 31 was likely to drop 28 per cent to 26 million pounds, on revenue of 165 million pounds.

That compared with analyst forecasts for pretax profit of 31.2 million pounds and revenue of 177.4 million, according to Reuters data.

Mulberry said retail sales over the Christmas period were generally in line with expectations but deteriorated over the last 10 weeks, including a drop in tourist spending at London stores. The firm trades from 11 stores in the capital as well as five outlets at airport terminals, and had been enjoying robust demand from visitors from Russia and Nigeria, as well as Asia.

Burberry, which has six London stores, declined to comment.

According to Britain's Office for National Statistics, visits to the country by overseas residents fell by 1 per cent in January, following two strong months in November and December.

CUTTING FORECASTS

Other luxury brands have also seen a slowdown in London trade as a result of fewer tourists.

"It's (shopper numbers) about 30 per cent less than last year, some days 60 per cent," said Paulina Rajnert, an administrative manager at Miu Miu's New Bond Street store, noting most of the reduction was due to fewer tourists.

Mulberry has recently been subject to takeover speculation. Earlier this month French luxury group Hermes denied a press report that is was mulling a bid.

Mulberry said retail like-for-like growth for the year was likely to be about 6 per cent, while wholesale sales were expected to be down 15 per cent.

The wholesale drop reflected a previously flagged move to limit the amount of stock going into lower quality wholesale accounts with the aim of growing the Mulberry brand's value in the longer term, as well as lower than expected in-season ordering.

The firm said the order book for autumn/winter 2013 was, however, building satisfactorily.

Analysts at Barclays cut their profit forecast for 2012-13 to the company guidance and their forecast for 2013-14 to £30 million from £39.8 million.

Separately today, French group PPR unveiled plans to rename itself "Kering" as part of its transformation from a retail conglomerate to a luxury and sporting goods group. – Reuters

TUNIS, March 22 — A Tunisian court has sentenced a rap singer to two years in jail in absentia for insulting the police in a case likely to fuel debate over free speech under the Islamist-led government.

The singer, known as Weld el 15, is on the run. Two of his associates, singers Mohamed Hedi Belgueyed and Sabrine Klibi, were in court when they received suspended sentences of six months each, a Justice Ministry source said today.

Belgueyed and Klibi were arrested earlier this month for a music video in which lyrics sung by Weld el 15 describe police as dogs and accuse them of using violence unjustifiably.

The two men asked the judge to pardon them before the sentences were handed down at yesterday's court proceedings.

"I was only using the language of the police. They have harassed me verbally and physically. As an artist, the only way I could answer them is through art. I gave them a violent art," Weld el 15 said in a video posted on Facebook.

Secular groups say freedom of creativity and expression are threatened under a coalition led by Tunisia's moderate Islamist Ennahda party, a charge the government denies.

In particular, they accuse authorities of failing to prevent attacks by militant Islamist Salafis on cultural institutions and individuals. Salafis disrupted several concerts and plays last year, saying they violated Islamic principles.

They also ransacked the US embassy in September during worldwide Muslim protests over an Internet video. — Reuters

‘The Twist,’ ‘Saturday Night Fever’ deemed recording treasures

WASHINGTON, March 22 — The US Library of Congress yesterday added 25 recordings to its national registry, including dance-craze inspirations such as Chubby Checker's tune "The Twist" and the "Saturday Night Fever" movie soundtrack, and pianist Van Cliburn's landmark Cold War Tchaikovsky performance.

This year's inductees represent a wealth of musical styles, from Broadway show tunes (Rodgers and Hammerstein's "South Pacific") to iconic 1970s rock and punk ("The Dark Side of the Moon" by Pink Floyd and The Ramones' "Ramones").

The inclusion of Cliburn's 1958 recording of Tchaikovsky's Piano Concerto No. 1 comes less than a month after the US pianist died in February at age 78.

Cliburn was the surprise winner of the first International Tchaikovsky Competition in Moscow in 1958, and his triumph and subsequent Soviet concert tour helped spur a brief thaw in US-Soviet relations.

Country music singer and two-term Louisiana Governor Jimmie Davis made the list with "You Are My Sunshine," as did avant-gardists Philip Glass and Robert Wilson for their five-hour opera, "Einstein on the Beach."

The National Recording Registry, created by Congress, began selecting and preserving recordings in 2002 to celebrate the richness and variety of the audio heritage of the United States.

Humorist, recording artist and stage star Will Rogers, blues great Janis Joplin and soprano Leontyne Price were included.

Rogers was cited for his Depression-era broadcast "Bacon, Beans and Limousines," Price was honored for "A Program of Song," while Joplin's "Cheap Thrills," her second release with Big Brother and the Holding Company, was lauded for its expression of both "desperation and endurance."

Spoken-word inductees included President Dwight Eisenhower's 1958 message to the nation via Atlas Satellite, and radio correspondent George Hicks' D-Day broadcasts from June 5-6, 1944.

Twenty-five recordings at least 10 years old are selected annually by an expert committee and suggestions from the public.

Parti politik, SPR sama-sama tunggu pengumuman bubar Dewan Rakyat

KUALA LUMPUR, 22 Mac — Dalam debaran menunggu pengumuman pembubaran Dewan Rakyat, parti-parti politik dan Suruhanjaya Pilihan Raya (SPR) melahirkan keyakinan bahawa mereka sudah bersedia untuk menghadapi pilihan raya umum ke-13 (PRU-13) yang ditunggu-tunggu itu.

Pengerusi Biro Publisiti dan Penerangan Barisan Nasional (BN) Datuk Ahmad Maslan (gambar) berkata jentera penerangan BN telah mengadakan sesi taklimat dan ceramah di seluruh negara, dan esok adalah yang terakhir di Sabah.

"Selagi belum pilihan raya kita kena terus bersiap dan terus bekerja tanpa henti sehingga kita tahu keputusannya. Segalanya sudah siap, kita hanya tunggu manifesto dan pengumuman calon sahaja," katanya ketika dihubungi di sini hari ini.

Ahmad yang juga Timbalan Menteri di Jabatan Perdana Menteri berkata manifesto BN biasanya akan dilancarkan selepas pembubaran Dewan Rakyat dan nama calon setiap kawasan Parlimen dan Dewan Undangan Negeri (DUN) akan diumumkan sebelum hari penamaan calon.

Sementara itu, Perdana Menteri Datuk Seri Najib Razak seperti biasa terus sibuk dengan jadual hariannya di Putrajaya manakala esok, beliau dijadual menghadiri beberapa program di Pahang.

Pengerusi SPR Tan Sri Abdul Aziz Mohd Yusof berkata SPR juga sama seperti pihak lain yang menunggu pengumuman pembubaran Dewan Rakyat.

"Sebelum masanya tiba, segala usaha penambahbaikan diteruskan SPR bagi memastikan PRU kali ini berjalan lancar termasuk membersihkan daftar pemilih," katanya.

Beliau berkata esok (23 Mac)  SPR masih mengadakan taklimat kepada Pegawai Pengurus dan Penolong Pegawai Pengurus peringkat Wilayah Persekutuan dan mengikut jadual taklimat itu adalah yang terakhir.

Seandainya PRU-13 diadakan dalam masa terdekat, jumlah keseluruhan yang layak mengundi ialah seramai 13,291,385 orang.

Bagi mereka yang mendaftar sebagai pemilih pada suku pertama tahun 2013 masih ada harapan untuk mengundi dalam PRU-13 jika Dewan Rakyat atau Dewan Undangan Negeri dibubar pada hujung April, dan hari pengundian diadakan selepas cukup tempoh 60 hari.

Presiden MCA Datuk Seri Dr Chua Soi Lek dalam laman twitternya pula berkata ahli MCA menunjukkan semangat yang tinggi dan bersedia untuk PRU-13.

Naib Presiden PAS Salahuddin Ayub berkata jentera parti tersebut telah berada pada tahap kesiapsiagaan yang terakhir dan segala persiapan bagi menghadapi PRU-13 sudah berada di tahap terakhir.

Beliau berkata PAS membuat persediaan sejak setahun lepas berikutan jangkaan bahawa pilihan raya kali ini merupakan yang paling sengit dalam sejarah tanah air.

Mengenai pemilihan calon, Salahuddin berkata senarai tersebut sudah 99 peratus selesai dan PAS bakal menampilkan calon yang terdiri daripada pelbagai latar belakang termasuk ulama, profesional, aktivis masyarakat dan tidak dilupakan calon wanita.  — Bernama

Skuad bimbingan Datuk K. Rajagobal menerima kejutan awal apabila Aiman Saleh Abd Al-Hagri menjaringkan gol pembukaan pada minit ke-11.

Harimau Malaya, dalam misi memburu dua pasukan teratas dalam kumpulan itu, bagaimanapun menyamakan kedudukan menerusi jaringan Azammudin Mohd Akil pada minit ke-26, hasil hantaran kemas oleh Mahalli Jasuli.

Skuad negara memperoleh peluang emas untuk mendahului saingan pada minit ke-40 tetapi sepakan deras penyerang berbisa Norshahrul Idlan Talaha tersasar selepas terkena pemain lawan.

Pada babak kedua, skuad negara rancak mengatur serangan bertali arus namun gagal menebusi benteng pertahanan lawan.

Bagaimanapun, percaturan Rajagobal membawa masuk Mohd Khyril Muhyameen Zambri membuahkan hasil apabila pemain itu menjaringkan gol kemenangan pada minit ke-75.

The Economist bagaimanapun mengatakan keputusan pilihan raya, yang paling ditunggu-tunggu dan dijangka paling sengit dalam sejarah Malaysia meragukan buat pertama kali.

"Kelebihan masih memihak kepada BN," menurut The Economist dalam artikel "Malaysia's looming election: Video nasties".

"Kawasan pilihan raya memberi kelebihan kepada penduduk luar bandar, dilihat lebih konservatif dari kawasan bandar yang kritikal, kosmopolitan dan mempunyai talian Internet," menurut majalah itu lagi.

Pemantau pilihan raya Tindak Malaysia mengatakan pada April tahun lalu satu undi pengundi luar bandar sama dengan enam pengundi bandar, menurut persempadanan pilihan raya sekarang.

Pengasas Tindak Malaysia PY Wong berkata gabungan pemerintah tersebut memenangi 112 dari 139 kerusi terkecil dalam Pilihan Raya 2008, dan memberi majoriti mudah di Parlimen dengan hanya 18.9 peratus dalam undi popular, dan jumlah kerusi tidak berubah untuk Pilihan Raya 2013.

Wong juga berkata pembahagian yang tidak sekata sertam manipulasi pensempadanan pilihan raya menyebabkan BN mampu menang 62 kerusi dengan hanya 6.2 peratus undi popular sahaja.

Perdana Menteri Datuk Seri Najib Razak pada Selasa mengumumkan Bantuan Rakyat 1Malaysia (BR1M) yang akan diterima oleh isi rumah yang mendapat pendapatan RM3000 sebulan atau lebih rendah, akan menerima bantuan tersebut setiap tahun.

The Economist berkata pakatan pembangkang — terdiri dari PKR, DAP dan PAS memenangi undi popular di Semenanjung Malaysia dengan 51 peratus pada pilihan raya 2008 dimana BN kehilangan majoriti dua pertiganya.

"Polisi afirmatif yang diperkenalkan 40 tahun lalu yang memberi kelebihan kepada kaum Melayu dan bumiputera lain menyebabkan minoriti Cina dan India tidak akan memberi sokongan menyeluruh kepada gabungan kerajaan itu," kata majalah itu lagi.

The Economist juga memberitahu Najib mempunyai kelebihan dalam memenangi pilihan raya, dan berbanding Pakatan Rakyat (PR).

"Dalam keadaan sekarang ia memihak kepada BN kerana mempunyai sumber yang banyak," katanya lagi.

The Economist berkata kurangnya berita dalam arus perdana mengenai pencerobohan Sulu di Sabah membantu BN dalam menghadapi pengundi Sabahan dari keturunan Filipina.

"Sememangnya pencerobohan itu bakal menyukarkan peluang BN di Sabah, tetapi kerajaan walau bagaimanapun akan mampu ditangani," kata majalah itu lagi.

The Economist menambah media arus perdana juga mengabaikan isu pembunuhan model Mongolia Altantuya Shaariibuu selepas kematian penyiasat persendirian P. Subramaniam selepas sakit jantung.

"Dalam pendapat umum, Najib dilihat sangat popular dengan 61 peratus menurut satu kajian," kata majalah itu lagi.

Tetapi The Economist juga menulis tentang tuduhan berulang mengenai salahlaku seks Ketua Pembangkang Datuk Seri Anwar Ibrahim mungkin menjejaskan popularitinya. Anwar semalam menyaman blogger pro-Umno "Papa Gomo" kerana menuduh beliau terlibat dalam video seks.

"Beliau (Anwar) masih mempunyai peluang untuk menjadi perdana menteri kerana pengakuan video yang menyebabkan kemarahan orang ramai mengenai rasuah di Malaysia," kata The Economist merujuk kepada video yang didedahkan oleh aktivis berpengkalan di UK Global Witness Selasa lalu, menunjukkan penyelewengan tanah di Sarawak melibatkan Ketua Menteri Tan Sri Abdul Taib Mahmud.

"Ramai rakyat Malaysia marah, tetapi mereka tidak terkejut," kata The Economist.

Hal ini berlaku ekoran kemelut bekalan air yang berpunca daripada dasar tiada kerjasama kerajaan negeri dengan syarikat-syarikat konsensi air di negeri ini, katanya kepada Bernama.

Beliau berkata dasar kerajaan negeri yang mahu menggagalkan operasi syarikat-syarikat konsensi telah menyebabkan tiada jaminan bekalan air yang mencukupi lalu mendorong syarikat pelabur membatalkan hasrat mereka melabur di negeri ini.

"Ini satu kerugian besar kepada rakyat Selangor, dalam keadaan kita sekarang hendak meninggikan pendapatan dan meningkatkan taraf hidup, peluang pekerjaan ini hilang begitu sahaja," tambah beliau.

Isa berkata sejak pakatan pembangkang berkuasa di Selangor, agenda utamanya ialah hendak mengambil alih Syarikat Bekalan Air Selangor (Syabas) dan Puncak Niaga Holdings Berhad yang hampir 10 tahun menguruskan air di negeri ini dengan baik kerana tiada gangguan politik sebelum ini.

Kata beliau, untuk menunjukkan Syabas bermasalah, kerajaan negeri sanggup membekukan perbelanjaan modal untuk syarikat itu dengan tujuan supaya Syabas gagal menguruskan air.

"Ini cara mereka hendak halang Syabas, supaya mudah untuk mereka mengambil  alih syarikat itu. Pelbagai alasan diberi kononnya Syabas ini kroni Umno tapi sebenarnya mereka hendakkan konsesi itu," kata Isa.

Beliau mempersoal bagaimana Syabas didakwa gagal, sedangkan kerajaan negeri mempunyai pegangan saham sebanyak 30 peratus dalam syarikat konsesi air ini.  

"Mereka mempunyai kuasa yang besar untuk menaik atau menurunkan tarif dan memberi bantuan modal. Sebenarnya bergantung kepada kerajaan negeri," kata beliau.

Menurut Isa, kemelut air yang berlarutan ini membuktikan bahawa kerajaan negeri tidak berupaya menguruskan industri air seperti yang dihasratkan mereka melalui penstrukturan semula.

Malah, beliau turut menyatakan kegagalan kerajaan negeri Selangor mengendalikan industri pasir sejak mengambil alih tampuk pemerintahan negeri pada tahun 2008, merupakan satu lagi contoh jelas, kerajaan negeri lemah dan tidak berupaya.

"Dahulu kerajaan hanya keluarkan pasir mengikut keperluan industri, sebab itu kita tidak jadikan pasir sebagai industri kita. Tetapi kerajaan Selangor sekarang ingin jadikan pasir sebagai industri utama negeri.

"Tetapi kalau mereka ikut apa yang disasarkan, Selangor tidak akan menjadi sebuah negeri yang aman, yang bersih, kerana akan wujud lombong-lombong pasir di seluruh negeri. Ini cukup berbahaya," kata Isa.

Sehubungan itu, Adun Batang Kali itu menganggap pemimpin kerajaan negeri Selangor bukan sahaja tidak mampu menyelesaikan kemelut air, tetapi cuba menutup kelemahan pentadbiran mereka.

Beliau berpendapat tawaran terbaharu untuk mengambil alih syarikat-syarikat konsesi air Selangor hanyalah gimik politik untuk mengalih perhatian rakyat kepada isu air yang sebenar.

"Kita sekarang menghadapi masalah air terawat, bukan masalah siapa hendak mengurus air. Bekalan air terawat akan berkurangan menjelang 2014 dan seterusnya, ini masalah kita dan kita perlukan loji baharu untuk menampung air", kata beliau.

Beliau menjelaskan bahawa persoalan utama dalam isu air sekarang ialah ada antara 34 loji yang sedia ada sudah melebihi tahap operasi maksimum serta tiada sumber air lain.

"Sebab itu kita perlukan Loji Langat 2 untuk menampung permintaan air di Lembah Klang merangkumi Kuala Lumpur, Putrajaya dan seluruh Selangor. Itu perkara asasnya. Bukan perkara siapa hendak jual air, atau siapa hendak jadi tauke air," jelas Isa.

Beliau menegaskan bahawa kerajaan pusat telah memikirkan soal loji ini sejak 16 tahun lalu. Pusat telah menganggar 10 tahun ke hadapan, bagaimana keadaan Selangor kalau tak ada loji baharu.

"Menteri Besar hendak alih pandangan rakyat, dan rakyat hari ini menyerang dia, saman dia kerana tidak membiayai bayaran air percuma kepada semua rakyat. Jadi untuk mengalihkan isu itu, dia lontarkan satu perkara yang seolah-olah orang akan lupa, apa yang dia gagal. Seolah-olah dia ada satu rancangan baharu untuk menyelesaikan masalah. Ini cuma permainan politik."

Isa juga mempersoal jika kerajaan Selangor tidak berjaya menguruskan pasir dan pentadbiran negeri hari ini, bagaimana hendak menguruskan industri air?

"Apa guna kita ada air percuma tetapi buka paip tak ada air? Kita terpaksa pikul air naik tingkat empat, tingkat lima, apa gunanya?" kata Isa.

"Kalau kita seorang pentadbir yang baik, kita tahu di mana kepentingan yang harus dijaga. Kita akan runding dengan kerajaan pusat dan rakyat untuk mendapatkan suasana menang-menang," tambah beliau. — Bernama

MARCH 22 — Sijah Ejut is a true blue Malaysian. She is an Iban tuai rumah (village head) who has lived her entire life on the outskirts of Sibu, Sarawak. She has never been out of the country, and her ancestors have lived on the land for centuries. You can't get any more Malaysian than that.

Yet, she only managed to register her citizenship and get an IC at the age of 27. And because of that, she couldn't finish her schooling. She couldn't even travel around freely because the police would stop and accuse her of being an illegal immigrant.

Sijah's isn't an isolated case. Almost the entire population of her village does not have ICs (or had trouble getting ICs) even though they are all Bumiputeras.

The infrastructure in her area is so bad that there are no roads, no electricity and no treated water. They wash in the river and light at night is only for those who can afford a generator.

These are people defined by the Constitution as sons of the land. These are people who are more Malaysians than Malays can ever dream of being.

Yet, they are treated as non-citizens. They are ignored and oppressed and denied even the basic human rights such as education.

Interviewing Sijah with writer Karim Raslan for a documentary series called "Ceritalah Malaysia", I could only watch and listen to her story.

I saw her try very hard, but unsuccessfully, to hold back her tears and sobs as she recalled how she was literally stopped from going to class because she wasn't considered a citizen.

She wept as she thought back of the time when she was 16 and felt her world was crumbling as she would never realise her dream of becoming a teacher.

How difficult would it be for the government to ensure that every rightful Malaysian citizen is treated with respect and dignity?

How difficult is it for the government to ensure that every rightful Malaysian citizen is given their rights as a Malaysian citizen?

How difficult is it for the government to ensure that every rightful Malaysian citizen is given the opportunity to play their part as a Malaysian citizen?

I doubt it is really difficult. Especially if the allegations made by so many prominent figures are true, that more than 700,000 people were given ICs under Project IC in 2012 alone.

Imagine how many undeserving people have been made Malaysian citizens if this has been going on since the 1980s and continuing till today.

And imagine Sijah Ejut's outrage now that she knows that one of Malaysia's most wanted men, Agbimuddin Kiram, and the rest of his mob could have been among those given ICs and made Malaysian citizens?
